Placement Application (pdf) - For organizations interested in receiving assistance through the Northwestern Law Public Service Program.

Student Evaluation (pdf) - Students evaluate their placement experience by completing this form for each pro bono placement they complete.

Student Time Log - Students participating in projects must keep track of the hours they work and report it on this form.

Supervisor Evaluation (pdf) - Supervisors evaluate their Public Service Program experience by completing this form at the end of the assignment.
