Completing the Daily Symptom Tracker Questionnaire
Complete the Northwestern University Symptom Tracker Questionnaire each day before arriving on campus.
The Symptom Tracker is alternatively available as a mobile app and can be downloaded via the Apple App or Google Play Stores.
Guidance on Reporting Positive COVID-19 Diagnoses
If a student receives a positive COVID-19 diagnosis, they should contact Northwestern Health Service (NUHS) at 847-491-8100 for further instructions. They should also contact Susie Spies Roth, Dean of Students. Additional student information can be found on the University's coronavirus website.
If an employee receives a positive COVID-19 diagnosis, they should notify Gwen Butler (Director of Environmental Health and Safety) and their department head or Law School HR. The impacted employee will be asked to provide a list of Northwestern community members who they have been in direct, proximate contact with over the last two weeks. Additional employee information can be found on the University's coronavirus website.
